Total knee Replacement (TKR) is an established technique to improve function and to relieve pain in osteoarthritis in Elderly patients. The success of TKR depends on achieving the proper ligament balancing and correct component alignment in order to restore optimal function and to guarantee the longevity of the implant. The incidence of joint disease continues to increase, correlating with an increase in the number of knee replacement cases.

Inaccurate implantation rates up to 30.1% have been reported using the conventional Knee Replacement techniques. Comparison between intra and extra medullar guide system has not shown advantages in reducing this number. Factors such as severe deformities, narrow canal and obesity limit the use of the conventional technique. The prosthesis mal-alignment has been reported as one of the factor for premature failure and early loosening.

Now a newer computer assisted technique developed known as iASSIST Knee which is an advanced surgical guidance system engineered to improve the accuracy and alignment of total knee replacement surgery. iASSIST computer assisted Knee helps to assist the surgeon in precisely aligning knee implants to each patient’s unique anatomy, providing a personalized fit and a truly tailored total knee replacement. Apart from this IASSIST computer assisted technique minimises the incision, potentially decrease emboli and risk of myocardial infarction (heart attack), reduces blood loss and allogenic blood transfusion, potentially provide faster rehabilitation, better function and decrease length of stay in the hospital.

No Pre-operative imaging is required with this technique therefore reduces waiting time for surgery and further lowering pre-operative costs.

Whereas, we have to drill pins into the bones for arrays for registering data into bulky computers in other navigation techniques, IASSIST is now pinless navigation technique with only Two Pods are needed. The equipment only involves small laptops size computer, Two Pods and other small instruments. Further this IASSIST technique allows validation of the cuts intraoperatively which increases the accuracy of alignment over other navigation techniques available. This technique proven to be much more accurate, fast, surgeon friendly and has given excellent possible clinical benefits for the patient with less complications.
